Social Prescribing

Everyone needs a bit of help from time to time. Social prescribing is a way of helping you deal with the things in your life that can make you feel unhappy or anxious. Addressing these things takes a little extra time and support. So rather than giving you a prescription for a medicine, social prescribing can help you to access a wide range of activities and support in your local community that can help you turn things around.

How it works
If your GP surgery is in Rhyl, Denbighshire, you can be referred by your GP or another health professional at your GP surgery to Vale of Clwyd Mind social prescribing service.

A trained link worker will arrange a meeting where you can talk about how you’re feeling and what you think might help to improve your well-being.

The link worker will recommend and help you to access a range of support and advice services to suit your needs, from housing and benefits support or advice to a local walking group.

Who it's for
This service could be helpful if you are out of work, have money problems, feel lonely, or have a physical health problem, are over 18 and struggling with your mental health.
